Reducing Avoidable Readmissions

The Institute for Healthcare Improvement (IHI) is offering a second Reducing Avoidable Readmissions by Improving Transitions in Care seminar on October 18-19 in Boston, MA.

Participants will learn practical, tangible steps to improve patient transitions between the hospital, home, and other points of care. Discussion will focus on how to develop more effective and reliable discharge plans, and how to increase post-discharge support within the community.
