Innovative Ways to Reduce Readmission Rates

Advance for Nurses recently ran a fascinating article detailing a readmission prevention program used by Geisinger Health in Pennsylvania.  The system uses technological advances including an electronic health record to monitor patients post-discharge.  This intervention led to a 44% reduction in 30-day readmissions when compared to a control group.
